Paraproteinemia and Kidneys – from Henry Bence Jones till Now Days

Urol Nephrol Open Access J 2016, 3(3): 00078 On addition of nitric acid, it effervesces, assumes a reddish hue, and becomes quite clear, but as it cools, assumes the consistency and appearance which you see. Heat reliquifies it. What is it?” Dr Henry Bence Jones tested the urine sample himself and presumed that the substance, defining unusual properties is “deutoxide of albumin”. Now we know that it was not albumin, but free light chains, and can presume that the patient suffered from multiple myeloma with cast nephropathy, which was not yet described at that time. This is in agreement with autopsy findings, revealed by Mr Shaw in January 1846. Autopsy report says: “soft and brittle ribs were noted to the extent that the osseous ribs crumbled under the heel of the scalpel”. No signs of kidney amyloidosis, which was well-recognized by that time as “lardaceous changes”, were found. Dr John Dalrymple, who performed histological examination, noted that mollites ossium was due to replacement of bones by a “gelatin-form substance of a blood red color and unctuous feel” and described nucleated cells which were present in large numbers in the affected bones. Illustrations from his report depict what we now days know as plasma cells.

The Role of the Kidney in the Catabolism of Bence Jones Proteins and Immunoglobulin Fragments
The role of the kidney in the catabolism of Bence Jones proteins, intact IgG molecules, and isolated L chains, Fab and Fc fragments of IgG, was studied. The proteins were purified, radioiodinated, and their survival times measured in nephrectomized, ureter-severed, and control mice. Active endogenous catabolism was the major factor in overall Bence Jones metabolism since excretion as proteinuri...

متن کاملClassification of Myeloma Proteins, Bence Jones Proteins, and Macroglobulins into Two Groups on the Basis of Common Antigenic Characters
Antisera to normal 7S gamma-globulin and to Bence Jones proteins permit the grouping of myeloma proteins (gamma and beta 2A types), Bence Jones proteins, and the Waldenström type macroglobulins into two fundamental antigenic groups. The antigenic determinants responsible for this grouping are common to all these proteins which fall in the general category of immunoglobulins. Antisera to Bence J...
متن کاملA note from history: the first biochemical test for detection of cancer.
The story of this epoch-making discovery as it unfolded in the 1840s has some similarity to a modern day clinical pathology conference. The place is the St. George’s Hospital in London and the participants are Dr. William MacIntyre (dates unknown), Dr. Henry Bence Jones (1814-1873), and Dr. John Dalrymple (1804-1852). A gravely ill patient of Dr. MacIntyre’s was admitted to the hospital with va...
متن کاملCxlvii. the Behaviour of Urinary Proteins in the Ultracentrifuge. by Arthur Sproul
THE question whether the proteins found in the urine in certain diseases have the same composition as the proteins in the blood is one of great importance. With the probable exception of two conditions, viz. orthostatic and Bence Jones proteinuria, it is believed that the presence of protein in the urine may be taken as a sign of impaired renal function. If the urinary protein is the same as th...
